For decades, Body Mass Index (BMI) has been the go-to metric for gauging whether someone's weight falls in a healthy range. But BMI has a well-known blind spot: it cannot distinguish between muscle and fat, and it completely ignores where fat is distributed on the body. What Is ABSI and How Was It Developed?
ABSI was introduced by Nir and Jesse Krakauer in a 2012 study published in PLOS ONE. The researchers analyzed data from the NHANES (National Health and Nutrition Examination Survey) population and found that waist circumference, when adjusted for BMI and height, is a strong independent predictor of premature mortality. The formula is:
ABSI = WC / (BMI^(2/3) × Height^(1/2))
where WC is waist circumference in meters, BMI is in kg/m², and height is in meters. The result is a dimensionless index that captures how much larger (or smaller) your waist is compared to what would be expected given your BMI and height.
Why Waist Circumference Matters
Abdominal fat - specifically visceral fat surrounding the internal organs - is far more metabolically dangerous than subcutaneous fat on the hips or thighs. High levels of visceral fat are linked to type 2 diabetes, cardiovascular disease, fatty liver disease, and certain cancers. Two people with identical BMIs can have dramatically different health profiles depending on their waist circumference. ABSI captures this critical distinction that BMI misses entirely.
How to Use the Calculator
Enter three measurements: your height, your weight, and your waist circumference. The waist should be measured at the midpoint between the lowest rib and the top of the iliac crest (hip bone), typically around the level of the navel. The calculator computes your BMI, your raw ABSI value, and - most usefully - your ABSI z-score, which tells you how many standard deviations your ABSI falls above or below the population mean for your age and sex.
Interpreting Your ABSI Z-Score
A z-score near zero means your abdominal size is typical for someone with your BMI and height. A positive z-score indicates more abdominal fat than expected - and the higher the z-score, the greater the associated mortality risk. The original Krakauer study found that individuals in the highest ABSI quintile had roughly twice the mortality risk of those in the lowest quintile, even after controlling for BMI, age, sex, and smoking status.
Below -0.868: Very low risk. -0.868 to -0.272: Low risk. -0.272 to +0.229: Average risk. +0.229 to +0.798: High risk. Above +0.798: Very high risk. These cutoffs are approximate and based on the original NHANES dataset.
ABSI vs BMI vs Waist-to-Hip Ratio
Each metric captures something different. BMI measures overall mass relative to height but ignores body composition. Waist-to-hip ratio (WHR) captures fat distribution but does not account for overall body size. ABSI adjusts waist circumference for both BMI and height, isolating the excess abdominal girth that drives health risk. Used together, these metrics give a much more complete picture than any single number.
Limitations
ABSI was validated on the U.S. NHANES population. Its predictive accuracy may differ in populations with different body compositions, such as certain Asian or African demographic groups. Additionally, ABSI does not replace clinical assessment - blood pressure, blood glucose, lipid panels, and lifestyle factors all contribute to cardiovascular risk.
